What UK law and guidance require before you can prescribe a cannabis-based product for medicinal use (CBPM).
Doctors
Who may prescribe
- A doctor on the GMC Specialist Register. Only a specialist may initiate treatment — GPs and trainees cannot.
- Working within your own specialty and competence.
- Indemnity that covers unlicensed cannabis-based products.
- Multidisciplinary review where your specialty expects it.
When it may be prescribed
- A diagnosed condition with a clinical need licensed medicines have not met.
- Licensed treatments already tried, and either ineffective or not tolerated.
- Full medical records reviewed, and the reasoning documented.
- A decision consistent with NICE NG144 and your specialist society's guidance.
Pharmacists, nurses and other prescribers
- You cannot initiate a CBPM. Starting treatment is reserved to a specialist doctor on the GMC Specialist Register.
- Once a specialist has started it, you can write the ongoing prescriptions under a shared care arrangement, where the patient is stable and you are confident in the decision.
- A patient-specific clinical management plan sets what you may adjust — dose, formulation — and when to hand back. The initiating specialist stays involved throughout.
- Schedule 2 rules still apply: each prescription is written fresh, on a controlled-drug form. These medicines cannot go on repeat.
If you haven't prescribed before
- The Medical Cannabis Clinicians Society (MCCS) runs training that gets you ready to prescribe — 1–3 hours. Shadowing opportunities are planned for later.
- You work alongside the large number of doctors already prescribing for Sanvivo in Germany.
